If you purchased a PIN prior to June 28th and have not used that PIN to schedule an appointment or if you cancelled your appointment and still have remaining scheduling opportunities, you should use the Contact Us option to request for an appointment to be scheduled using that PIN.

Once again, please use the 'Contact Us' option on the new site to make changes to your appointment that was originally made on this site or to use a PIN purchased that you have not used yet or has remaining scheduling opportunities to avoid paying for the service again. Alternatively, you can call the telephone service and speak with an operator. There will be a charge for the call. Please consult the new website for the different telephone options and associated charges.
